This winter we are asking all of our parishioners to completely update their contact information.  We attempted to fill in the gaps last winter with a short form left in the pews, but unfortunately, we didn't capture all of the data that we were hoping to.  As a result, we are working together to complete a total overhaul of our census- with a goal to have it completed by the end of February.  Accurate and complete contact information is necessary so that we can serve you better in the future.

As a staff, we have worked together to generate a completely new census form which more efficiently collects the contact information that matters in 2020.  This includes updated entries for individual e-mail addresses and cell phone numbers as well as demographic information, special needs, and sacraments.

In order to complete your updated census with us, you have several options:

1. You may complete a paper census form and return it to either office, in person or via mail.  A paper census form will be mailed home to you this January with your annual charitible donation tax letter.  We will also make copies of the new census form available in the narthex of both OLQP & STM.  CENSUS FORM

2. You may complete an online census entry for EACH member of your family living at home with you.  The online form is short and easy to complete, but it does need to be completed for you, your spouse, and each of your children (if applicable).  The data collected here is secure and confidential.  ONLINE CENSUS FORM

3. You may call the office at 381-4200 (STM) or 244-3010 (OLQP) and speak to Robin, Jen, or Amanda to update your contact information via telephone.

4. You may stop in either office at OLQP or STM when the office is open, between 8:00 am - 1:00 pm, Monday through Thursday.  Robin, Jen and Amanda always look forward to seeing parishioners in the office and will happily sit with you to help you complete your updated census form.
